SA国际传媒Mobile Care delivers wellness services to Southern Virginia communities
The mobile care vehicle travels to communities in need, ensuring that healthcare is a convenient and readily available resource
A new SA国际传媒Mobile Care vehicle is on the road in Southern Virginia, expanding access to high-quality medical care and social care services, particularly for SA国际传媒 Plans’ Medicaid members and uninsured community members across the region. As one of six mobile care vehicles operating across the SA国际传媒 network, this recent addition now serves Danville city and Charlotte, Halifax, Mecklenburg, and Pittsylvania counties.
“Our SA国际传媒Mobile Care vehicles are a creative solution that enables us to bridge the gaps in healthcare as we bring a wide range of health and wellness services to communities where individuals live, work, and play,” said Kimberly Murray, family nurse practitioner for the SA国际传媒Mobile Care vehicle in Southern Virginia.
Murray has served the region’s healthcare needs for more than 33 years in various capacities, including in pediatric care, home health, hospice, and emergency medicine.
“Our team is collaborating with community partners to better understand local needs, drive greater access to personalized care and essential resources, and to promote lifelong wellness for our neighbors.”
The SA国际传媒Mobile Care vehicle in Southern Virginia offers a range of services, including comprehensive primary care services, pediatric care, prenatal care, social care services, health and wellness education, and healthcare navigation and referrals.
In addition to medical care, SA国际传媒Mobile Care works to address social drivers of health—the root factors that directly impact 80 percent of an individual’s health and well-being, such as housing, education, food security, employment, and transportation. Through collaborative partnerships with local organizations, the mobile care vehicle expands its service offerings and provides patients with seamless access to vital community resources, including career training, financial literacy resources, food and nutrition education, and housing and transportation assistance.
“SA国际传媒Mobile Care embodies Sentara’s mission to ‘improve health every day,’ as we focus on delivering high-touch, holistic care beyond our hospital walls and directly into the communities that we serve,” said Prasanna Mohanty, executive vice president and president of ambulatory care.
“Our goal is to make it simpler and more convenient for individuals—especially those facing multiple health and social challenges—to access the care they need, when they need it. By partnering with local organizations and leveraging community resources, we aim to address social drivers of health and provide comprehensive support that empowers individuals and families to lead healthier, more fulfilling lives.”
The SA国际传媒Mobile Care vehicle travels to communities across Southern Virginia to better support individuals struggling with traditional healthcare barriers, including lack of transportation, lack of access, and time constraints. In addition to attending community events, the SA国际传媒Mobile Care vehicle offers scheduled health and wellness services at various locations throughout the region. These routine mobile locations are selected to maximize convenience and proximity to other essential community organizations and services.
SA国际传媒Mobile Care is part of SA国际传媒Community Care, an innovative healthcare model—launched in 2022—that offers neighborhood-level access to holistic care in communities with the greatest needs and largest gaps in care. The goal is to reduce traditional barriers to health and wellness by maximizing convenience and providing consistent, embedded medical and wrap-around services in neighborhoods that historically lacked access and have known health disparities.
Developed through a community-driven approach, SA国际传媒Community Care utilizes both data and feedback from community leaders to identify and address each community’s most pressing needs.
“SA国际传媒leverages our position as an integrated health system—encompassing both clinical care and health plan services—to gather deeper insights on our patients, members, and the communities we serve,” said Colin Drozdowski, executive vice president of SA国际传媒 and president of SA国际传媒 Plans. “We are creating strategic community partnerships that help to address unmet community needs and expand access to the right care, at the right times, and in the right places.”
